Healthcare Provider Reporting Requirement

Under Utah Code 78B-5-618, healthcare organizations or providers using third-party services to provide patient access to medical records must submit a statement to DOPL. This statement must include the provider's name and contact details for the third-party service, including the company’s name, phone number, fax number, email address, website portal (if applicable), and the mailing address for medical record requests.
