Third-Party Medical Record Services


Under Utah Code 78B-5-618, healthcare organizations or providers using third-party services to provide patient access to medical records must submit a statement to DOPL. This statement must include the provider's name and contact details for the third-party service, including the company’s name, phone number, fax number, email address, website portal (if applicable), and the mailing address for medical record requests. The organization or provider is responsible for ensuring information is current and accurate.

To streamline compliance, the Division of Professional Licensing offers a submission process for acquiring the necessary information. Additionally, the division maintains an alphabetical index of submitted information, which can be found by selecting the menu item labeled Medical Record Services Index. This process promotes transparency and accessibility in overseeing third-party participation in medical record fulfillment.
