That would depend. The statute requires an entry for each company with which a healthcare organization contracts to fulfill medical record requests. If each location contracts with a different medical company, then the healthcare organization would need to make an entry for each location. If multiple locations contract with the same company for medical record requests, the healthcare organization only needs to submit an entry to DOPL for each medical record company and specify what locations this company serves.

- If a patient or other provider can submit a medical records request to you or the healthcare organization you worked with prior to retiring to receive their records, then no action is required.
- If a patient or other provider must submit a medical records request to an outside company to fulfill their request, then yes, you or the healthcare organization you worked for will need to complete this requirement. Contact your former healthcare organization to determine whether they have fulfilled this requirement on your behalf.
