Fingerprints

ANNOUNCEMENT
The Division of Professional Licensing is discontinuing its in-house fingerprinting services. If you are applying for a license that requires a fingerprint background check, follow the "How to Submit Fingerprint Cards" instructions.

Many DOPL license require applicants to submit fingerprints. These fingerprints are used by the Division for a fingerprint search through the files of the Utah Bureau of Criminal Identification (BCI) and the Federal Bureau of Investigations (FBI).

How to Submit Finerprint Cards:

  1. Submit Your Application: Applicants must complete and submit all other requirements for initial licensure.
  2. Access Vendor List: Once your application is submitted, you will receive an email with a list of authorized third-party fingerprinting vendors, along with an Authorization Form.
  3. Schedule Your Appointment: You will then schedule and complete your fingerprinting requirement by providing the vendor with your Authorization form.

Privacy Act Statement

Prior to submitting fingerprints, you must read and acknowledge the Privacy Act Statement, fbi.gov/services/cjis/compact-council/privacy-act-statement . You will find a Criminal History Disclosure Statement in your applicant, that includes a link to this form and an affidavit acknowledging you have read the information and consent to the background check process. This affidavit is a required part of your application, whether you obtain your fingerprints at the DOPL offices or submit physical cards from a third party vendor.
