FSMB will contact you once the report has been submitted to the state of Utah.
This is considered public information the division is permitted to provide to anyone requesting the contact information. This is a method of contact for patients attempting to obtain their own medical records. Two contacts are required by statute. Typical contacts include: records department, office manager, clinic, or clinic staff.
If you will be prescribing any controlled substances you will be required to hold the federal DEA license as well as the Utah controlled substance license. If you are applying online, the controlled substance application is separate from the medical application. If you are applying manually, there is an affidavit within the medical application to request the controlled substance license.
