Apply for a Health Facility Administrator License

If you are currently licensed as a health facility administrator in a state, territory, or district of the United States you may qualify for licensure by endorsement. Click here for additional information.

The following requirements must be met:

Pass the National Association of Boards of Examiners for Nursing Home Administrators (NAB) Examination
Be ready to upload an Affidavit of Completion of AIT Preceptorship (application page 4)
Request, from your school, Official Transcripts to be sent to the Division, OR request, from the state, where you are currently licensed, to send a verification of your license, to be sent to the Division, OR be ready to upload verification, supported by tax forms (W2, 1040 Schedule C), of 8,000 hours of Health Facility Administrator Experience
