Frequently Asked Questions (FAQs)

The Division of Professional Licensing does not accept license verification requests via email. If the receiving state accepts the verification electronically follow the steps below:

Electronic

Go to https://secure.utah.gov/llv/search/index.html
Enter your name or license number
Select the license you require the verification for
Select "Obtain a License Verification"
Enter receiving email address & pay processing fee

Standard Mail

If the receiving state requires the verification to be sent by standard mail please submit a written request by mail along with the $20 processing fee to the address listed below:

Verification payments need to be received with the manual request either in person or via mail. Please make all checks or money orders payable to Division of Professional Licensing.

Enclose with the application complete information with respect to all circumstances and the final result for EACH and EVERY incident:

Personal account of the incident
Police report(s)-You will need to contact the police department that responded to the incident
Court record(s)-in the local jurisdiction where the incident took place
Probation/Parole officer report(s)

If you are unable to obtain any of the records required above, you must submit documentation on official letterhead from the police department and/or court indicating that the information is no longer available.
