Apply for a Massage Assistant In-Training License

The following requirements must be met:

Ready to upload the completed Assistant in-training instructor page.
Ready to upload your instructor's qualifications, including 6,000 hours of licensed massage therapy experience.
Ready to upload the board approved curriculum, including a list of the resource materials, that will be used.
