Pharmacists - 30 hours of Continuing Education.
Pharmacy Technician - 20 hours of Continuing Education.
An active PTCB orExCPT certification shall fulfill all CE requirements for a pharmacy technician, except for immunization/vaccine related topic hours for those engaging in the administration of immunizations or vaccines.

You must submit the correct "Change Form" in order to make a change to PIC, Designated Representative, or Responsible Individual. Click on the licensing tab on the left-hand menu, select "Additional Forms" and select the appropriate change form.
Complete and submit the Pharmacy Surrender Form .
