Apply for a Substance Use Disorder Counselor License (Utah Applicant)

The following requirements must be met:
- Official transcripts documenting completion of an associate degree or higher from a regionally accredited institution of higher learning.
- Official transcript documenting completion of prerequisite courses from a regionally accredited institution of higher learning in human growth and development and in general psychology.
- Official transcript documenting completion of a substance use disorder program from a regionally accredited institution of higher learning than include 200 hours of substance use disorder education, a 200-hour practicum, and 2 hours of suicide prevention training.
AND
- Official score reports of passing the NCC AP Exam, NCC AP MAC Exam, ADC Exam or the AADC Exam. More information can be found on our Exam Information website.
