Controlled Substance Database

Welcome to the Utah Division of Professional Licensing Controlled Substance Database Practitioner Training. This video covers an introduction and description of the Utah Controlled Substance Database, Utah CSD act, law and regulations, CSD account creation, patient and prescriber dashboards, proxy information and medication disposal.

You will have three sets of questions  that you will answer in the training/examination. At the end of the exam, you will be prompted to enter your CS license number (MUST use a dash) and the last 4 numbers of your social security number. The system will authenticate you and then update the CSD exam date stored in the state licensing system. Please allow a few minutes for this process.
