Proxy Access
Licensed Practitioners with an active Controlled Substance license may request proxy access for appropriately trained designees. While there is no limit on the number of designees a practitioner may have, the practitioner is responsible for each designee, as well as their searches performed.
Practitioners must notify the Division immediately of any disassociation of a designee . See below for instructions.
Initial Proxy Access – Apply Online:

To Add Proxy Access To An Existing Designee:
- Complete an Affidavit of Practice (page three of the physical application) or a complete application for EACH practitioner that access is needed for.
- Include a legible copy of your Driver License, State Issued ID, or Passport
- Submit documentation (view Document Submission Options below)
To Remove Proxy Access:
- Complete the Disassociation of Designee form for each designee that no longer needs access.
- Submit documentation (view Document Submission Options below)
